Admission Process
Admission Criteria: The Talisman Foundation is equipped to welcome clients who meet the following requirements:
- Diagnosis of a mental health condition
- Within the age range of 18 to 54 years
- Deemed suitable for rehabilitation by their psychiatrist
- Determined to be a fit for Talisman’s rehabilitation program by our specialized admissions team.
*Kindly note, individuals with a history of substance or alcohol misuse must have achieved sobriety for a minimum of 3 months before the admission process can commence.
Referral Process: Initiating a referral to the Talisman Foundation can be undertaken by potential clients, their family members, or professionals such as social workers, psychiatrists, or counselors. This referral process entails:
- Filling out an admission application
- A thorough review by our admissions team
- Agreement on fees reflective of our high-quality care and commitment to non-charitable status
- An invitation to view our facilities at the Talisman Foundation
- A concluding admission interview
Accommodation: We offer both single and double occupancy rooms designed to cater to the needs of our distinguished clientele.

4 Step Process
We’ve designed a simple 4-step process to make your application quick, easy, and hassle-free. Just follow the steps, and you’ll be done in no time!
Step #1: Application Criteria
Please review the criteria for admission below to determine if the patient qualifies for the Talisman Foundation:
The Talisman Foundation Is a Psycho-social rehabilitation center catering for persons with a Psychiatric/Dual diagnosis.
Who can apply for admission?
- Adults (18 to 54 years) who have been diagnosed by a Psychiatrist and treated for a Psychiatric illness with or without a substance abuse component, who need assistance in re-integrating back into the community.
- Males or Females.
Who qualifies for admission? Those who:
- Have a Psychiatric diagnosis eg Schizophrenia, BMD, personality disorder etc.
- Have a Psychiatric diagnosis with a substance abuse problem.
- Have problems with social skills.
- Are prepared to fully participate in the program.
- Are able to care for their basic needs eg bath, dress, clean teeth, keep environment clean etc.
- Are clinically stable.
Who does not qualify?
- Persons with intellectual disabilities.
- Persons with physical handicaps.
- Persons who are acutely psychotic.
- Persons who have epilepsy without a psychiatric diagnosis.
- Persons who need acute psychiatric treatment.
- Persons who need a detoxification program drugs/alcohol.
- Persons who need frail care.
- Those in need of permanent placement.
- Person 55 years and over.
Application Process:
- On the application form the section titled “referring Psychiatrist report”, must be completed by a Psychiatrist who has seen the applicant within the last month. Failure to comply with this will result in the form not being processed.
- On receipt of the completed assessment document the applicant will be assessed for suitability by the Psychiatrist and the Psychiatric Nurse.
- If the application is successful, the applicant will be informed and if a bed is not available immediately the applicant will be placed onto the waiting list.
- On acceptance for a placement at the Talisman the person who will be responsible for the finances must contact the financial manager for our fee structure.
- If the application is unsuccessful the applicant will be informed telephonically.
- Should an applicant, family member or significant other wish to visit the Talisman before admission they are welcome to make an appointment.
Admission Requirements:
- The person must have an identity document.
- As discussed with the financial manager one month’s full rent must have been paid before admission.
- A month’s supply of medication, including any injection they take must be available along with a valid prescription.
- An arrangement must be made for medication, which includes injections, to be delivered to the Talisman Foundation on a monthly basis.
- The person must have an appointment with their Psychiatrist.
- The financial agreement section of the assessment document must be completed by the person who is responsible for payment.
- The responsible family member and the person being admitted must be aware of the General Terms and Conditions of the Talisman Foundation.
- Please note that persons coming from home who have a substance abuse problem will undergo a multi drug test and should they test positive will not be admit.
Step #2: Application Form
This form contains the patient’s personal details and links to step #2 and step #3.
Step #3: Psychiatric Report
This form should be completed by the patient’s family doctor or psychiatrist – This gives our team more insight into the patient’s history and what they are struggling with.
Step #4: Financial Report
We aim to highlight the importance of ongoing financial support for our patient’s, activities and projects. At the Talisman Foundation, though we are not classified as a charity, our dedication to providing outstanding care and rehabilitation services remains steadfast.
Once all 4 steps are completed, your application is then fully submitted and our team will review the application forms and contact you within 48 hours.
